Only for North African nationals: Signed and dated document stating that you have not resided nor carried out your main activity (studies, work, etc) for more than a total of 12 months over the last five years in the European Union.
You will have to write a letter yourself stating that condition. This could be an example:
"I, (your full name) hereby confirm that I have not resided nor carried out my main activity (studies, work, etc) for more than a total of 12 months over the last five years in the European Union”
